If you received a letter to provide your insurance policy declaration page, please follow the steps outlined below to provide proof of insurance.

How to Provide Proof of Insurance

Please note that we are unable to accept an insurance ID card as it does not contain all the required insurance information. To provide your insurance policy declaration page:

  • Visit and enter the Reference ID and PIN listed on your letter
  • Fax your insurance policy declaration page to 1-877-495-8109
  • Mail your insurance policy declaration page to:

      Fort Worth, TX 76124

Insurance Requirements

As per your loan agreement, your collateral (vehicle or home) needs to be continuously insured against physical damage losses during the entire term of your loan. To help us process your insurance in a timely manner, please refer to the following primary insurance requirements:

  • Continuous coverage with no lapses
  • McGraw-Hill FCU must be named as the lien holder or loss payee
  • Comprehensive and collision coverage (for physical damage) with a deductible no greater than $1,000
